Missed Appointment

Appointments canceled with less than 48-hour notice or no-shows will be charged a $75 fee.

Medication Refill Policy

Refills require up to 48 business hours.

Controlled substance refills may require a visit or drug screen.

Early refills on controlled medications are not permitted.

Weapons of any kind are not permitted on clinic premises, regardless of licensure.

Video surveillance is used in public and clinical areas for patient and staff safety.
No surveillance occurs in bathrooms or private changing areas.


Spravato & Brain Health Monitoring Room Notice
Certain treatment rooms are continuously monitored for safety, medical oversight, and compliance during Spravato administration and Brain Health Monitoring sessions.
