Controlled Substance Prescribing Policy
Unwind Your Mind Psychiatry & Wellness recognizes that controlled substances may be necessary for the treatment of conditions such as ADHD, anxiety, insomnia, and other mental health disorders. However, due to the potential for misuse, dependency, and diversion, strict prescribing policies are followed:

Initial Evaluation

A thorough psychiatric evaluation is required before prescribing any controlled substance.

An evaluation does not guarantee that a controlled substance will be prescribed

Review of past medical records, prescription monitoring program (PMP/AWARExE), and when necessary, urine drug screening will be completed.

Informed Consent

Patients must sign a Controlled Substances Agreement acknowledging their understanding of the risks, benefits, and responsibilities.

Prescription Monitoring

All controlled substance prescriptions are monitored through the state PMP.

Random urine drug screenings may be requested at any time to ensure compliance.

Medication Refills

Refills will only be provided during scheduled follow-up appointments.

Early refill requests, lost or stolen prescriptions, or medication misuse may result in the discontinuation of controlled substances.

Treatment Plan

Controlled substances are prescribed as part of a comprehensive treatment plan, including therapy, lifestyle interventions, and non-controlled alternatives when appropriate.

Discountinuation

If a patient violates this policy, shows signs of misuse, or fails to follow up as scheduled, controlled substances may be tapered and discontinued.
