FMLA, Disability, & Document Policy
Purpose
To ensure accurate, ethical, and timely completion of all administrative paperwork, Unwind Your Mind Psychiatry and Wellness requires that most documentation be completed during a scheduled appointment. This enables the provider to review treatment progress, clarify medical information, and ensure that all documentation meets both clinical and regulatory standards.
Fee Schedule
Type of Documentation Description / Example Fee (if completed outside of an appointment) Standard FMLA / Work Accommodation Forms 1-2 page forms for short-term leave or workplace modifications $75
Extended FMLA / Disability Forms: Multi-page or detailed forms requiring review of records and functional assessment. $200
Letters / Custom Documentation: Return-to-work or school letters, housing accommodations, fitness-for-duty, or treatment verification. $50-$75
Rush Request forms are needed within 48 hours of receipt. +$40

Appointments Required for Completion:

All FMLA, disability, and related paperwork must be reviewed and completed during a scheduled appointment whenever possible.

If paperwork is completed during an appointment, no additional paperwork fees apply beyond the standard appointment charge.

Requests Outside of Appointments:

Patients who request that paperwork be completed outside of an appointment may do so at the provider’s discretion and will be subject to the above administrative fees.

Payment must be received before any out-of-appointment paperwork is completed.

Eligibility:
-
Patients must be established in care for a minimum of 4–6 months and actively engaged in therapy alongside medication management before eligibility for FMLA or disability documentation.

Turnaround Time:

Standard processing for out-of-appointment paperwork is 7–10 business days after payment and receipt of all required information. Rush requests (within 48 hours) will include an additional $40 fee.

Provider Discretion:

Completion of any form or letter is at the provider’s discretion and based on clinical appropriateness and documentation of treatment progress.

Delivery:

Completed forms may be uploaded to the patient portal, faxed, or provided via secure email upon request.
